So...here's my scenario, all yee experts, please chime in...

The last two main rides (Tour de Houston and MS150) I have discovered that after about 40-45 miles, I start to develop some slight discomfort in my left knee.  It's about the inner top portion of it.  Not muscle related...seems to be more of a joint/bone related thing.

It's not a pain until about oh...mile 60-70.  Then in a certain position while spinning, it is a bit of a sharp, local pain...a little annoying. 

In the TdHouston I thought maybe it was because I spent a good part of the ride on the aerobars...but then again, same thing happened during the MS ride where I didn't do that. 

What do you guys think?  Fitting? Age? Stroke? (no, not heart stroke) Or just inability to ride?

A picture of the knee with a dot on the part that hurts will explain what is happening to you better.

there are a ton of factors when it comes to knee pain.

check your equipment first.   is your saddle straight/level?  are your shoes in good condition?  and check your cleat on that shoe;  sometimes they twist a bit.

if it's only one side & it's not equipment related, it's probably due to a slight difference in leg length, either actual (skeletal) or effective (muscular).  easy fix:  http://www.jensonusa.com/store/product/PE406Z06-Lemond+Lewedge.aspx.  pain above the kneecap is often caused by a saddle that's too low.  you might need to 'lengthen' the other leg so you can bring the saddle up to a height that makes the pain go away.

I agree with Dr. V. I had the same problem until I went for a bike fitting(I should have done this when I got my bike but oh well) The guy found that my left knee was very unstable because whenever I would pedal, my left knee would rub against the upper tube of my bike making it very painful after 40 miles as well(I didnt even noticed till I saw it myself in the mirror). REdo your cleats, they do change over time, and you may be looking to try footpads. This guy was very nice with me and put a specialized ball of the feet pad on my left side (shh! for free!)and that resolved my asymmetry problem. Actually, next specialized order, I am hoping I can be able to order those...

Guess I'll have to make another trip to Sugar then and see what they say.  Brett took a look after Tour de Houston and saw everything alright...but two long rides in a row with the same problem, I'm hoping it's a fitting problem rather than an issue with my knee overall.

drink more water Wink just kidding, Im not an orthopedic, but based on my experience, you might wanna lower you seat post by a centimeter, my right knee used to click when it was too high, so i dropped it just a little bit and now its great. Its all about alignment, pressure points, and stuff. Your butt cheek might have something to do with your knee pain too. Maybe you have too much meat on one side, or a pinched nerve or something. Talk to your doctor and tell them I sent you.

If I tell them you sent me.. will it get me a discount!? haha. Could it be the way im pedaling? maybe im not fully extending my leg all the time which is giving it a sharper pain. Is there a proper way to pedal? uhh...

I dunno, i depends... I wanna say everyone's body is different, but in someway we really aren't. Just tweak your positions, make adjustments, and see what works. Trial and error. You have to investigate your problem.

pedaling many times is a reason for pain. Ask someone to look at you pedaling to see if your form doesn't look weird. You can also look at your at the mirror, you just need a trainer for that.
