Fall 2009 Waiver

If you are planning on racing this fall you must fill out the following waiver in its entirety: www.cougarcycling.com/files/memberinfo.doc including a copy of your medical insurance card with it and get it to Kris Young (kgyoung@uh.edu) at MINIMUM one week before your intended race. 

You may email a scanned copy of the waiver and your card (preferred method) or drop it by my office (slide under door if I am not there)

DO NOT SEND YOUR WAIVER TO ANYONE ON THE TEAM.  THIS YEARS WAIVER IS ASKING FOR WAY TOO MUCH PERSONAL INFORMATION THAT TEAM SHOULDN'T KNOW!!!  INSTEAD EMAIL YOUR WAIVER DIRECTLY TO RASHAD SANDERS RASANDER@CENTRAL.UH.EDU WITH THE HEADING COUGAR CYCLING WAIVER AND EMAIL ME (KGYOUNG@UH.EDU) WHEN YOU SEND HIM YOUR EMAIL.

We will make up a separate information sheet for you to fill out for the the team.
